How to add and manage patient diagnoses in WoundScribe, from opening a chart to confirming a new ICD-10 code.
WoundScribe opens on your active patients and today's encounter status, so you can track work across Not Started, In Progress, and Notes Ready at a glance. Here's how to add a diagnosis to a patient record.
- From the Patients view, filter by date range, ownership, or status, or type a name or MRN into the top search bar.
- Start typing (e.g. "Mary") and pick the matching record from the results to open the chart.
- Open the Diagnoses tab to see every active, proposed, and past diagnosis. The header shows the current counts (for example, 26 active, 0 proposed, 0 past).
- Click Add diagnosis in the top right to open the search panel.
- Type an ICD-10 code or description, like "hyper", to surface every matching option.
- Select the diagnosis you need, such as D73.1 Hypersplenism, and it's added straight to the record.
The list refreshes with the new count (now 27 active), and your entry appears at the top marked Active and sourced as Manual, confirming the addition landed cleanly. AI-proposed diagnoses show up in the same list, ready for you to confirm or edit alongside anything you add by hand.
